Our responsibilities
Victor J. Dongo, DMD, PA is required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach of your unsecured PHI occurs.
How we may use and disclose your information
- Treatment — to provide and coordinate your dental care, including with specialists and dental laboratories.
- Payment — to bill and collect payment from you, your insurance plan, or another payer.
- Health care operations — for quality review, training, scheduling, and running our practice.
- Appointment reminders — to contact you about appointments and treatment options.
- As required by law — including public health activities, health oversight, legal proceedings, law enforcement, and to avert a serious threat to health or safety.
Other uses and disclosures, including most marketing uses, require your written authorization, which you may revoke at any time in writing.

Sending health information securely
Protected health information should only be shared through approved secure channels: in person at our office, by phone with our team, or through a secure method our staff provides to you. Ordinary email, text messages, and the general appointment request form on our website are not intended for sensitive medical information. Please do not include diagnoses, medications, records, or insurance ID numbers in those channels.
Complaints
You may file a complaint with our office using the contact information below, or with the U.S. Department of Health and Human Services Office for Civil Rights at hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
Changes to this notice
We may change the terms of this notice, and the changes will apply to all information we have about you. The current notice is available in our office and on this website.
